Community Health Assessment

Mahoning County Public Health is collaborating with its partners to conduct a Community Health Assessment (CHA). The CHA is a local health survey conducted every three years that helps identify the most significant health concerns for those who reside in Mahoning and Trumbull counties. The input and the information provided by Mahoning and Trumbull county residents (18 years of age and older) is valuable. The information collected from the 2025 CHA will help create and guide action plans to address the identified major community health issues. This process encourages connecting, coordination and working together toward a common goal to help strengthen the programs, services and resources that create a healthy community.
